At Ease Health and Wellness accepts credit card, debit card, HSA and FSA for payment, and we require all patients to keep a card on file to pay for any services received, due at time of service. If you need a receipt to submit to your insurance company for reimbursement, please let us know.

REMINDER: If you cancel or no-show to any appointment with less than 24 hours notice, a $60 charge will be made to your account per our financial policy agreement.

NEW PATIENTS: Please ensure all online intake forms are complete 12 hours prior to first appointment. This information helps us provide you with the best care possible, and Dr. Chara reserves the right to reschedule your appointment if the paperwork is not completed.
